We came across this a couple of times recently and the supplier swapped the switches out each time. It drove the engineer mad the first time it happened, he was at the end of a long day working out-of-hours and thought he was seeing things.
Strangely the replacements didn’t have the problem with same IOS & same config, so we’d assumed it was a wiring defect rather than a software bug.
the problem seems to occur only in model revisions <= E0.
